Sonourethrography Versus Retrograde Urethrography Findings In The Assessment Of Anterior Urethral Strictures In A Tertiary Institution In Lagos, Nigeria

Abstract

Imaging has an important role in the evaluation of stricture diseases of the male urethra.  Precise and reliable assessment of anterior urethral strictures in terms of location, number, length and degree of spongiofibrosis is very important in its management. This information will help the urologist determine the choice of therapy and prognosis.


Objectives


This study compared the efficacy of sonourethrography (SUG) with retrograde urethrography (RUG) in assessment of anterior urethral strictures.


Materials & Methods


This was a comparative, cross-sectional, and descriptive study of SUG and RUG in the assessment of anterior urethral strictures in 69 subjects.


Results


The average age of participants was 49.8±17.0 years. Urethral trauma was the major cause of stricture in this study, (58.0%). Total number of strictures diagnosed was 82 and 80 on SUG and RUG respectively. A penobulbar stricture was missed on RUG but diagnosed on SUG. Multiple loose penile strictures seen on RUG was seen on SUG as multiple mucosal tags. Strictures were characterized as regards their site, number, length and grade of spongiofibrosis. Associated findings included diverticula, mucosal tags, and calculi. The overall sensitivity of SUG was 97.3% and specificity was 83.7%.


Conclusion


SUG was found to be equally efficacious in the detection of anterior urethral strictures when compared with RUG. SUG was found to be more reliable and accurate in characterization of anterior urethral strictures in terms of length, width and periurethral abnormalities.


Advances in knowledge


The diagnosis and follow up patients with anterior urethral stricture without the use of ionizing radiation.
